Where can I find my Appointment code?
ATTENTION: If you made an appointment by phone or via MyHUB, you cannot cancel or modify an appointment via a form. Please go back to the My Appointment page and select the channel you chose when you initially booked your appointment.
You'll find your code in the email you received after booking your appointment.
It consists of the word 'Erasme' followed by 16 digits (e.g. Erasme xx xxxxx xxxxxxx xx).
If you can't find the email in question, you can request to receive your code by email by clicking on the link 'I no longer have my appointment code' on the modification or cancellation pages, or by clicking here.
